Job Title: Transition of Care Registered Nurse (Remote – Northern Virginia Region)
Contract Type: Contingent Labor
Location: 100% Remote (Candidates must reside in the Northern Virginia (NOVA) Region)
Duration: 10 weeks, 6 days (with potential for extension)
Schedule: Full-time | 40 hours per week | Monday–Friday, 8:00 AM – 5:00 PM EST
Pay Rate: $41.76/hour
Position Overview:We are seeking a dedicated Transition of Care Registered Nurse (RN or LPN) to support member transitions from inpatient acute care settings to home or skilled nursing facilities (SNFs). This role is critical in ensuring continuity of care and positive health outcomes during high-risk transitions. The position is fully remote but requires candidates to reside within the Northern Virginia (NOVA) region.Key Responsibilities:

- Provide transition of care support for members discharged from inpatient acute care settings.
- Coordinate care plans with members, families, and providers to ensure safe discharges to home or SNF settings.
- Collaborate with interdisciplinary teams to address member needs post-discharge.
- Perform follow-up assessments and documentation to monitor member progress and ensure service delivery.
- Utilize clinical judgment to identify and mitigate risk factors impacting member recovery.
- Active RN or LPN license with strong clinical experience in medical/physical health.
- Proven expertise in Transitions of Care and Discharge Planning.
- Excellent communication, organizational, and documentation skills.
- Strong understanding of community resources, care coordination, and patient advocacy.
- Reliable high-speed internet and the ability to work independently from a home office.
- Employer-provided: Laptop, keyboard, mouse, headset
- Candidate-provided: One (1) additional monitor
- Duration is intended to cover an FTE’s leave of absence but may be extended based on business needs.
- Candidates must be located in the NOVA region and work Eastern Time Zone hours.
