Job Title: Integrated Case Manager – Population Health
Location: Michigan (100% Onsite)
Pay Rate: $47.58  per hour
Schedule: 40 hours per week, 8-hour shifts
Contract Duration: 13 weeks (with potential for direct hire for candidates with a Bachelor’ s degree)

 
Job Summary:
Our client is seeking an Integrated Case Manager for Population Health to join their patient-centered care team. This role is responsible for assessment, care coordination, facilitation, and advocacy to support patients with complex medical and social needs. The position focuses on optimizing patient safety, quality of care, and cost-effective outcomes through collaborative case management.
Key Responsibilities:
  • Conduct comprehensive assessments of patient and family needs, including biomedical, psychological, social, and functional factors.
  • Develop personalized care plans to enhance patient outcomes and experience.
  • Engage patients and families in the care process through advocacy, education, and resource facilitation.
  • Utilize motivational interviewing and problem-solving techniques to help patients overcome barriers to care.
  • Provide counseling and support for treatment decisions and end-of-life planning, including Advance Care Planning.
  • Ensure seamless care transitions between settings, coordinating with providers and support services.
  • Advocate for appropriate service delivery within the patient’ s health plan benefits.
  • Collaborate with multidisciplinary teams to co-manage patients with complex needs and facilitate care conferences.
  • Continuously evaluate patient response to care plans, modifying as needed.
  • Participate in process improvement initiatives aimed at reducing risk and improving care delivery.
  • Intervene in cases involving child abuse, domestic violence, elder abuse, or sexual assault when necessary.
Work Environment & Additional Details:
  • 100% onsite position in Michigan.
  • On-site training will last one week, candidates must learn quickly and work independently.
  • Case Ratio: 1: 21 – 25, but not all patients require direct case management— risk factors determine patient interactions.
  • Candidates with utilization management, therapy, or telephonic case management experience are NOT a fit for this role.

Job Requirements

 

Required Skills & Experience:

  • 3-5 years of recent acute inpatient case management experience.
  • Strong verbal communication and documentation skills.
  • Exceptional customer service, interpersonal, and teamwork abilities.
  • Strong problem-solving, analytical, and decision-making skills.
  • Proficiency with Epic EMR and Microsoft Office Suite.
  • Knowledge of Medicare/Medicaid regulations, case management principles, and medical ethics.
  • Understanding of social determinants of health and their impact on patient well-being.
  • Experience with community resources and services for diverse populations.

Education Requirements:

  • Required: Nursing degree or Master’s in Social Work (MSW).
  • Preferred: Bachelor’s degree (required for direct hire consideration).

Required Certifications & Licenses:

  • Registered Nurse (RN) with a valid, unrestricted State of Michigan license
    OR
  • Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license
  • Basic Life Support (BLS) Certification (AHA)
 

Preferred Experience (Not Required):

  • Prior experience in discharge planning, home health care, community health, or managed care.
