Job Description
Job Title: Remote Field Case Manager – Licensed (Virginia)
Pay Rate: $49.09/hour
Location: Remote/Field – Must reside in Virginia (Northern Virginia Area preferred)
Schedule: Monday – Friday, 8: 00 AM to 5: 00 PM (No weekends or holidays)
Travel: Local travel required (up to 40%); mileage reimbursed
Equipment Note: One additional monitor required
Work Environment: Remote-based with in-field visits; high-speed internet connection required for home office setup
Shift: Full-time 40 hrs/wk
Salary: up to $49.09/hr.
Position Overview:
We are seeking a licensed Case Manager residing in Virginia to support our Severe Mental Illness (SMI) Medicaid population. This role combines remote work with field-based responsibilities, including face-to-face assessments in members’ homes, primarily throughout Northern Virginia (NOVA). The Case Manager will play a key role in creating, monitoring, and adjusting care plans to meet member needs while collaborating with interdisciplinary care teams.
Key Responsibilities:

Conduct clinical assessments (telephonic and in-person) per regulatory timelines

Develop, implement, and monitor individualized case management plans

Perform in-home visits to assess and support member well-being

Document assessments, contacts, and interventions with precision and timeliness

Collaborate with care teams, caregivers, providers, and members’ support networks

Facilitate interdisciplinary care team (ICT) meetings

Utilize motivational interviewing and health education tools to drive positive health outcomes

Integrate behavioral and long-term care services to ensure continuity of care

Identify and address barriers to care, providing coordination and support

Educate members on wellness programs including asthma, depression, and chronic disease management

Job Requirements
Required Qualifications:

Must reside in Virginia and hold an active, unrestricted clinical license in the state

Valid driver’s license and reliable transportation for home visits

1–3 years of experience in case management, disease management, behavioral/medical health settings, or managed care

Ability to work in a fast-paced environment and manage a consistent case load

Exceptional organizational, communication, and computer skills

Strong documentation skills and attention to detail across multiple systems
Education & Licensure (One of the following):
-
Licensed Clinical Social Worker (LCSW)
-
Licensed Professional Counselor (LPC/LPCC)
-
Licensed Marriage and Family Therapist (LMFT)
-
Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) – from an accredited program
-
Registered Nurse (RN) – with Bachelor's degree preferred
-
Bachelor’s/Master’s degree in Social Work, Psychology, Gerontology, Public Health, or related field
-
Additional certifications such as CCM, CHEP, or APSW are a plus
Preferred Experience:
-
Experience working with the SMI population

Knowledge of Medicaid guidelines and behavioral health integration

Proficiency in using electronic documentation systems and case management software
